Graduation Application - Continuing Studies

NOTE: There are separate forms for Regular Studies and Continuing Studies programs. Please ensure you select the correct form.

In order to receive a credential for their chosen Continuing Studies program, all students must apply for graduation. Students are encouraged to apply for graduation during the semester in which they complete their program requirements.

To complete your order, please download and fill in the Continuing Studies Graduation Form. Then upload a PDF or image file of your completed form, select your quantity (one unit per credential applied for) and pay for your purchase.

Note: Requests made with an incomplete form or without a form attached will not be processed.
