NOTE: There are separate forms for Regular Studies and Continuing Studies programs. Please ensure you select the correct form.

Use this if your original Continuing Studies credential (Post-Degree Diploma, Diploma, Certificate, etc) was lost or damaged.

To complete your order, please download and fill in the Continuing Studies Parchment Replacement Request form. Then upload a PDF or image file of your completed form and pay for your purchase. Only one replacement credential is available.

Note: Requests made with an incomplete form or without a form attached will not be processed.

Requests are non-refundable and non-transferable.
