Use this if your original credential (Post-Degree Diploma, Post-Degree Certificate, Degree, Diploma, Certificate, etc.) was lost or damaged.

To complete your order, please download and fill in the Parchment Replacement Request Form. Then upload a PDF or image file of your completed form, and pay for your purchase. Only one replacement credential is available.

Note: Requests made with an incomplete/incorrect form, or without a form attached will not be processed.

Requests are non-refundable and non-transferable.
